29 CFR 1910.23(c)(1)(i): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) and was not provided with a toe board wherever, beneath the open sides, and whenever persons can pass:    Employees were required to perform work on elevated work areas which lacked guardrail protection. Employees were exposed to falls of 4ft or more to a lower surface while performing work related tasks.    a. On the Mezzanine there were Open-sided floors four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards. This exposed employees to a fall hazard.     b. Temporary Work Platform- Employees are required to stand on stacked pallets (approximately 4ft 9in) to perform work. This exposed employees to a fall hazard.    c. Interior Satellite Office- Employees are required to gain access to and retrieve items from the top of the building. This exposed employees to a fall hazard.    Abatement documentation is not required for this item.

Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section.     Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o). Employees were exposed to the hazard of being caught in a ingoing nip point.       Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section.  The belt and pulley assembly on the high pressure washer was unguarded.  Employees were exposed to the hazard of being caught in a ingoing nip point.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
