The employer did not institute a training program and ensure employee participation in the program:  a) Custom Fab, Inc., Gainesville, GA - An employee operating the cutting saw #1 to cut metal was exposed to noise at a time-weighted average of 92.3 dBA, which exceeded the action level of 85 dBA. The sample was taken for 407 minutes. Noise training was not conducted, on or about 5/5/15.   b) Custom Fab, Inc., Gainesville, GA - An employee abrasive blasting was exposed to noise at a time-weighted average of 97 dBA, which exceeded the action level of 85 dBA. The sample was taken for 159 minutes. Noise training was not conducted, on or about 5/5/15.   c) Custom Fab, Inc., Gainesville, GA - An employee operating the threader #11 was exposed to noise at a time-weighted average of 93.2 dBA, which exceeded the action level of 85 dBA. The sample was taken for 453 minutes. Noise training was not conducted, on or about 5/5/15.   d) Custom Fab, Inc., Gainesville, GA - An employee operating the cutting saw #2 to cut metal was exposed to noise at a time-weighted average of 89 dBA, which exceeded the action level of 85 dBA.
