29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.    On 5/5/15, in the production area, employees were exposed to tripping hazards as product (pipe) was staged around work stations requiring employees to step over the pipes as they guided finished product with hoists to the middle aisle.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.  On 5/5/15, at the Gainesville facility, employees were performing Authorized work in that they were changing saw blades but were not trained as Authorized employees and were not using and protected by lockout measures when performing this work.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    On 5/5/15, at the Gainesville facility, employees were exposed to amputations as moving and spinning parts used at the facility were not guarded. Machines include 24x240 Leblond Lathe - Serial  #67-638.                                                              In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.    On 5/5/15, at the Gainesville facility, the employer did not ensure specific written Lockout procedures implemented at the facility addressed all hazards and requirements. Deficiencies included:    1  Disconnect locations   2  Identification of all energies and magnitudes of energies   3  No lock removal/transfer procedure    Custom Fab, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(ii)(b), which was contained in OSHA inspection number 978943, citation number 1, item number 2 and was affirmed as a final order on 6/20/2014, with respect to a workplace located at 109 5th St., Orlando, Florida.
