29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. At the jobsite located at 1187 Gage, Topeka, Kansas: Employees engaged in commercial roofing activities were observed working on a low slope roof and were exposed to fall hazards of approximately twelve (12) feet from the eave of the structure to the ground in that fall protection systems were not utilized. L.J. Beck Roofing and Guttering, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 314464769, citation number 1, item number 2 and was affirmed as a final order on 03/13/2012, with respect to a workplace located at 8441 NW Prairie View Road in Kansas City, Missouri.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At the jobsite located at 1187 Gage, Topeka, KS: Employees engaged in commercial roofing activities on a low sloped roof were exposed to fall hazards of approximately 12 feet from the eave of the building to the ground below in that the ladder used to access the roof was not extended at least 3 feet above the eave and was not secured to the structure. L.J. Beck Roofing and Guttering, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 314464769, citation number 1, item number 3a and was affirmed as a final order on 03/13/2012, with respect to a workplace located at 8441 NW Prairie View Road in Kansas City, Missouri.
