On or about April 30, 2015, at the facility of FWD/Seagrave Fire Apparatus, located at 105 E. 12th Street Clintonville, WI 54929, employees in the Body Weld Area were exposed to fall hazards of approximately 12 feet from working on top of truck bodies while installing cabinets and covers for the rear portions of fire trucks.

On or about April 30, 2015, at the facility of FWD/Seagrave Fire Apparatus, located at 105 E. 12th Street Clintonville, WI 54929, employees were exposed to an unguarded Accurpress press brake that was not adequately guarded to prevent employees from getting any part of their body in the point of operation. c. On or about April 30, 2015, at the facility of FWD/Seagrave Fire Apparatus, located at 105 E. 12th Street Clintonville, WI 54929, employees were exposed to an unguarded Accurpress press brake that was not adequately guarded to prevent employees from getting any part of their body in the point of operation. d. 29 CFR 1910.141(b)(1)(i): Potable water shall be provided in all places of employment, for drinking, washing of the person, cooking, washing of foods, washing of cooking or eating utensils, washing of food preparation or processing premises, and personal service rooms. a. On or about April 30, 2015, at the facility of FWD/Seagrave Fire Apparatus, located at 105 E. 12th Street Clintonville, WI 54929, employees were not provided potable water in the restroom facilities for washing of the person. Water is supplied from a well on the premises that the Wisconsin DNR had deemed non-potable after an inspection dated June 27, 2001.
