29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: a) Sky Flyer On or about April 29, 2015, employees were exposed to fall hazards while constructing the Sky Flyer amusement ride. An employee was standing at the tower base which is 5' from the gravel ground below. Employee was wearing a harness but was not tied off to an anchor point.
