29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     a)  Mint Room, Packaging Line 60, Shanklin Wrapper (MN F-1, SN F99142).  On or about April 21, 2015, employees reached under the infeed conveyor to unjam the chain, sprocket and drive rod system to enable the conveyor to resume motion after it stopped multiple times during production.  Employees were exposed to finger amputation and laceration hazards.  The employer did not require that employees utilize established lockout/tagout procedures for this task.  Employees were not protected from unexpected start-up of the conveyor when their hands were in the hazard area, nor was the chain and sprocket guarded to prevent exposure.       b)  Mint Room, Packaging Line 60, Life Saver Machine.   Employees who performed setup were not protected from the unexpected start-up of the machine's wheels and other rotating parts and were exposed to finger and hand amputation and laceration hazards. The employer did not require that employees utilize established lockout/tagout procedures for this task, nor was the machine adequately guarded to prevent exposure.      There is no abatement documentation or certification required for Instance a).    Abatement documentation is required for Instance b) in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a) Mint Room, Packaging Line 60, Life Saver Machine and Shanklin Wrapper.  On or about April 21, employees assigned to perform un-jamming and setup servicing tasks were not required to apply energy controls in the following sequence:    1) turn off and shut down the machines,  2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s),  3) affix lockout or tagout devices to each energy isolating device,  4) verify that isolation and deenergization of the machines or equipment have been accomplished.    Employees were exposed to caught-in hazards likely to result in amputation or lacerations to hands and fingers.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program were understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees:    Mint Room, Packaging Line 60, Life Saver Machine and Shanklin Wrapper.  On or about April 21, 2015, employees who performed servicing and maintenance activities such as setup and unjamming were exposed to amputation and laceration hazards to hands and fingers in that the employer did not ensure that lockout/tagout training was provided to all "authorized" and "affected" employees to ensure that the purpose and function of the energy control program were understood by employees and that the knowledge and skills required for the safe application, usage and removal of the energy controls were acquired by employees.          Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    Mint Room, Packaging Line 60, Tray Former Machine.  The drive motor belt for the top plunger was not guarded to prevent access to the ingoing nip point between the belt and pulley.  Employees were exposed to a finger and hand amputation hazard.    There is no abatement documentation or certification required for this item.

On or about April 21, 2015, the chain and sprocket wheel for the infeed conveyor's drive rod system, located beneath the conveyor table, was not guarded.  Employees were exposed to finger amputation hazards during production.  There is no documentation or certification required for this item.
