The unguarded and/or inadequately guarded equipment includes; but is not limited to, the following:           a. The hot metal circular saw located in the roll shop was not adequately guarded.  Employees are exposed to flying chips and sparks.          b. The American (vertical) drill and boring mill was unguarded. Employees are exposed to potential rotating parts.            c. Rockford cutter and milling bed, located in the machine shop, was unguarded.  Employees are exposed to potential struck- by hazards.     d. The Giddings and Lewis boring, drilling and milling machine shop, was unguarded. Employees are exposed to potential struck-by hazards.          e. The platen bed of the conveyor system of the Kearney & Trecker milling machine was unguarded, exposing employees to in-going nip points and/or crushing hazards.         f. The work rest on the right side of the pedestal grinder  located in the MTC shop was not properly adjusted. The opening measured approximately one-half inch.    b. The damaged conduit and controls includes, but is not limited to, the following:    a. The junction box located on the northwest side of the run-out table located in the Cast department had exposed live wires.      b. The three (3) push button controls for the AP Roller Bearing machine located in the Equipment Rebuild department, was defective. Employees who use the controls to actuate the machinery were exposed to electrical shocks.

Fall hazards include, but are not limited to, the following:     a. On or about 05/05/2015, the portable platform with trumpet base, exposed employees to a fall hazard of approximately 10 feet.      b. On or about 05/06/2015, the pump house work platform of bag house four and bag house five, was unguarded.  Employees were exposed to a fall hazard of 9 feet.       c.  On or about 05/21/2015, employees who access the tops of the tundish cars, were exposed to a fall  hazard of 9 feet.      d.  On or about 05/22/2015, the west-end of the shot blaster work platform, located in the IBBI department, had a 6 to 9 inch gap at the edge of the platform.  Employees  were exposed to struck by hazards from tools fall from the platform above.     e.  On or about 07/16/2015, the cranes numbers 2;22;14;15;17;19;31;29; and 27 did not have eye bolts or anchor points for tying off.  Employees are exposed to a fall of approximately 25 feet.        f. On or about 07/17/2015, the landing for crane number 159 located in the Roll shop was not installed.  Although the company provided JLGs for use, the lift did not allow access to crane.  Employees climbed over end truck of an additional crane,  exposing employees to a fall of approximately  25 feet.     g.  Crane #30, located in the run-out area of the Cast department, did not have end railing on the north-end of the truck exposing employees who access the area to a fall hazard of approximately 25 feet.       h.  On or about 05/19/2015, employees  stood on the HRME transfer cars to stamp the heat number on the ends of the steel bar, were exposed to falls of approximately 15 feet. Employees use the cross over to access the hydraulic parts and equipment room. Employees were exposed to falls onto the (hot) blooms/slabs which travel on the run-out rolls below the platform. Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs:       a. On or before 05/05/2015 wooden stairs, located on the west side in the Tundish rebuild were not slip- resistant and the noising was cracked and splintered.    b. On or before 04/30/2015, the employer failed to ensure that the riser and run of the work platforms with fixed staircases used by employees to access the gear box on top of the Precision Sizing Mill (PSM) stands, located in the Roll Shop area, were consistent throughout. On May 28, 2015 the Meta Cut VI circular saw, located in C-building, was not adequately guarded.  Employees were observed reaching around the side of the guard to retrieve cut samples of bar stock, and were exposed to the rotating blade.     d.  On May 28,2015 the Sutton straightener, located in the C-building at the HRM, was unguarded. Employees remove and replace the internal guide (both straight and/or concaved) on the front and back side of the bar straightener were exposed to caught between or struck-by hazards.    e. The Chevalier CNC lathe, located in the small machine shop area, was inadequately guarded.  Employees were exposed to rotating parts.
