29 CFR 1910.219(i)(2): Shaft couplings were not so constructed as to present no hazard from bolts, nuts, setscrews, or revolving surfaces: (a) Inspection Department: On or about 4/30/15, the employer did not ensure that the revolving universal joints, with cotter pins, on the power transmission shafts for winders #1 and #2 were adequately protected by a guard or safety sleeve.
