29 CFR  1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   a) Production Area: On or about December 21. 2023, the employer did not develop an energy control program consisting of energy control procedures, training, and periodic inspections for maintenance employees who are performing work on equipment such as, but not limited to, winders, which had not been isolated or rendered inoperable prior to starting work.

29 CFR 1910.212(a)(1) One or more methods of machine guarding was not            provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points, rotating parts and rotating bolts of fabric:  a) Inspection Machine by Maintenance Area: On or about December 21, 2023, employees were exposed to hazards on the rear side where the safety trip cord did not extend to where the employes were working.   b) Inspection Machine #4: On or about December 21, 2023, employees were exposed to hazards on the rear side where the safety trip cord did not extend to where the employes were working.   c) Deleted  d) Production Area Laminator #2: On or about December 21, 2023, employees were exposed to hazards on the laminators rear side where the safety trip cord did not extend to where the employes were working.   e) Production Area Laminator #3: On or about December 21, 2023, employees were exposed to hazards on the laminators rear side where the safety trip cord did not extend to where the employes were working.   f) Production Area Laminator #4: On or about December 21, 2023, employees were exposed to hazards on the laminators rear side where the safety trip cord did not extend to where the employes were working.
