29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by ingoing nip point(s): The machine guard for the drive intake on the conveyor belt, running from the main level to the upper level of the facility, did not completely cover the drive movement on the upper level, creating a nip point hazard for employees retrieving boxes from the conveyor.

29 CFR 1910.219(e)(3)(ii): All guards for inclined belts were not arranged in such a manner that a minimum clearance of seven (7) feet was maintained between belt and floor at any point outside of the guard: The conveyor running from the main level to the upper level was not guarded to a height of 7 feet where it crossed the hallway leading to the parking lot on the left hand side of the building. The conveyor created a head bump/caught-in hazard for employees.
