29 CFR 1926.150(c)(1)(viii): Portable fire extinguishers were not inspected periodically and maintained in accordance with maintenance and use of portable fire extinguishers, N.F.P.A. No. 10A-1970: (a) At the project on or about April 8, 2015, and at times prior thereto, a fire extinguisher was not charged and it was not mounted. There were paper, rags, and scrap lumber onsite. Potential ignition sources included defective electrical equipment and motorized equipment running at the worksite.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. (a) At the project on or about April 8, 2015, and at times prior thereto, one yellow (SJTW 3X12 AWG) 50 foot extension cord was not connected to the plugs. This caused outer insulation and inner 3 wires to be directly transmitted to the plug joints or terminal screws. The extension cord powered a 110 VAC tile cutter used to cut ceramic tiles, exposing workers to the hazards of contacting electricity.
