At this commercial water treatment site under construction, employees were using the temporary electrical panel to provide power to electric powered tools. Unused opening on the underside of the temporary electrical panel were not effectively closed.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. At this water treatment facility construction site, employees worked in a trench which ranged from 7 to 19 feet with ladders which were not of an adequate height for the safe egress of employees. Employees were seen climbing up hill to get out of this trench.
