29 CFR 1910.176(b): 29 CFR 1910.176(b): The storage of material shall created a hazard. Items such as bags, containers, bundles, etc., were not stored in tiers and stacked, blocked, interlocked and limited in height so that they were stable and secure against sliding or collapse.    At the Pier 1 Imports warehouse located at 2200 Heritage Parkway, Mansfield TX, storage racks had damaged vertical and horizontal upright components. Employees were exposed to falling objects and struck-by hazards created by potential structural failure and collapse of storage rack systems.    1. Horizontal Support  Located at 56-21-402-6   2. 29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    At the Pier 1 Imports warehouse, located at 2200 Heritage Parkway, Mansfield, Texas, forklift training did not consist of all of the required elements of operator training, such as, but not limited to, a combination of formal instruction, practical training, and evaluation of the operator's performance, exposing employees to struck by and crushed by hazards.    Pier 1 Imports was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(2)(ii),which was contained in OSHA inspection number 987149, citation number 1, item number 1a and was affirmed as a final order on February 4, 2015 with a final abatement date of February 27, 2015 with respect to a workplace located at 14900 Trinity Blvd, Fort Worth, Texas 76155.    Pier 1 Imports was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(2)(ii),which was contained in OSHA inspection number 987164, citation number 1, item number 1a and was affirmed as a final order on February 4, 2015 with a final abatement date of February 27, 2015 with respect to a workplace located at 2200 Heritage Parkway, Mansfield, Texas 76063.
