29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.   On or about May 4, 2015, on the north side of the house, employees working on a roof installing asphalt shingles were using pneumatic nail guns and did not wear eye protection, exposing themselves to the hazard of being struck in the eye or face by flying debris.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a.  On or about May 5, 2015, on north side of a residential house on a 5/12 pitch roof, three employees installing asphalt shingles to the roof were not protected from a 8 feet 8 inch fall hazard by the use of a personal fall protection system.  b.  On or about May 5, 2015, on west side of a residential house on a 5/12 pitch roof, an employee installing flashing was not protected from a 10 feet fall hazard by the use of a personal fall protection system.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a.   On or about May 4, 2015, on the west side of the house, the ladder used to access the porch roof did not extend 3 feet above the upper landing surface, exposing an employee to an 10 feet fall hazard.
