On or about May 19, 2016, an employee at the jobsite was exposed to an overhead hazard, in that the employee was underneath two employees who were performing re-roofing operations without a protective helmet, exposing himself to being hit by falling debris.

On or about may 19, 2016, on a 4:12 pitch roof, two employees performing roofing operations were not protected from a 9-foot 6-inch fall hazard by use of a guardrail system, safety net system, personal arrest system or any alternative fall protection.
