29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  On or about 04/23/2015, at 1530 Joy Lane, Decatur, GA; employees performing tarring operations were exposed to falling onto building material debris including but not limited to brick, block, wood, and metals that were not properly cleaned up from the ground.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  On or about 4/28/2015, at 1530 Joy Lane, Decatur, GA; where two employees were observed performing roof tarring activities on a 77 foot wide flat roof and were exposed to:  a. 33 foot fall to the scaffold walking boards below, or  b. 43 feet, one and one half inch (1.5") fall to the lower level if beyond scaffold below, or   c. approximately 27 foot impalement hazard below on the tips of the ladder jack scaffolds where the employees were not wearing fall protection and were not connected.  Dyer Roofing & Sheet Metal Co., Inc. was previously cited REPEAT for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 314206913, citation number 2, item number 1 and was affirmed as a final order on 2/25/2011, with respect to a workplace located at 1221 Lawrenceville Highway, Lawrenceville, GA.
