29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: David Price Metal Services, Inc. - Norwalk, Ohio: On or about May 4, 2015, employees operating the Cincinnati Millacron CNC machine (s/n 89-4190406-1) were exposed to injury associated with the machine's rotating parts. The employer did not assure the rotating cutting head was guarded.

29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided: David Price Metal Services, Inc. - Norwalk, Ohio:On or about May 5, 2015, the durability of parts designed to enclose and protect electrical equipment of the coolant pump located at the Cincinnati Milacron CNC machine (s/n 89-4190406-1) exposed employees to electrical hazards in that the junction box cover was damaged and removed from the parts it was designed to protect.
