OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards:  A)    David Price Metal Services, Inc., located at 360 Eastpark Drive, Norwalk, OH 44857: On or about June 15, 2017, the employer did not ensure that the Telemecanique pendant control, used with the EMH hoist in the area of the Wotan 1 horizontal boring mill machine, was properly maintained. The employer did not ensure that all of the pendant control buttons were properly labeled, exposing employees to struck-by hazards in the case when the crane could be moved in the unintended direction.  No additional abatement information is needed for this item.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  A)  David Price Metal Services, Inc., located at 360 Eastpark Drive, Norwalk, OH 44857: On or about April 26, 2017, the employer did not ensure that employees were protected from fall hazards when standing on the open-sided work platform while operating the Wotan 1 horizontal boring mill machine located in the boring mill area. Employees were exposed to a fall hazard of approximately 7 feet from the floor of the platform to the bottom of the pit below.

29 CFR 1910.184(d): Damaged or defective sling(s) were not immediately removed from service:  A)  David Price Metal Services, Inc., located at 360 Eastpark Drive, Norwalk, OH 44857: On or about June 15, 2017, the employer did not ensure that the chain sling, used with the EMH hoist in the area of the Wotan 1 horizontal boring mill machine, was removed from service due to defective hook. The hook was not outfitted with a safety latch, exposing employees to struck-by hazards in the case when the load and/or the lifting device could be dislodged from the hook.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  A)  David Price Metal Services, Inc., located at 360 Eastpark Drive, Norwalk, OH 44857: On or about June 15, 2017, the employer did not ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, the rotating chuck of the Clausing drill press (Model # 2276, Serial # 509265) located in the machine shop. Employees were exposed to amputation hazards.
