OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: A) David Price Metal Services, Inc., located at 360 Eastpark Drive, Norwalk, OH 44857: On or about June 15, 2017, the employer did not ensure that the Telemecanique pendant control, used with the EMH hoist in the area of the Wotan 1 horizontal boring mill machine, was properly maintained. The employer did not ensure that all of the pendant control buttons were properly labeled, exposing employees to struck-by hazards in the case when the crane could be moved in the unintended direction. No additional abatement information is needed for this item.

Employees were exposed to a fall hazard of approximately 7 feet from the floor of the platform to the bottom of the pit below.

The hook was not outfitted with a safety latch, exposing employees to struck-by hazards in the case when the load and/or the lifting device could be dislodged from the hook.

Employees were exposed to amputation hazards.
