29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):    a)  On or about February 24, 2015, and at times prior, TOLL CO, L.P., as a controlling employer, did not ensure that employees of South Roofing, LLC (subcontractor), were protected from falls of greater than 6 feet while they performed roofing work on a multi-story home.  Employees worked without adequate fall protection measures in place and TOLL CO, L.P. had not conducted regular inspections of the South Roofing worksite.      b)  On or about February 24, 2015, and at times prior, TOLL CO, L.P., as a controlling employer, did not ensure that employees of South Roofing, LLC (subcontractor), were protected from eye injuries while they performed roofing work on using pneumatic nail guns.  Employees worked without adequate eye protection measures in place and TOLL CO, L.P. had not conducted regular inspections of the South Roofing worksite.
