29 CFR 1910.24(h): "Railings and handrails." Standard railings shall be provided on the open sides of all exposed stairways and stair platforms. Handrails shall be provided on at least one side of closed stairways preferably on the right side descending. Stair railings and handrails shall be installed in accordance with the provisions of 1910.23. a) The steps going from the shipping department to the loading dock was not equipped with a standard guard railing system to prevent employees to falls.

29 CFR 1910.178(n)(4): The driver shall be required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing. a) Operators of powered industrial vehicles (forklfits) did not travel with the load trailing in that while lifting three totes the forward view was obstructed.
