29 CFR 1926.754(e)(5)(i): Except as provided in 1926.760(c), metal decking was not laid tightly and immediately secured upon placement to prevent accidental movement or displacement.  A. Foundation Steel, LLC worksite located on the Anthony Wayne Bridge (Near 48 Stratton St.), Toledo, Ohio: On or about May 1, 2015, the employer did not ensure the steel Stay-In-Place (SIP) fabricated metal forms were immediately secured after being placed on the ladder supports. Multiple SIP deck pans were set on the ladders, not secured, thus resulting in an employee falling approximately 35.00 feet to the ground below after stepping on the unsecured deck pans resulting in serious injuries.
