Employees did not control all hazardous energy sources such as, but not limited to, electrical before performing activities such as, but not limited to, changing blades. Employees were exposed to amputation hazards.

Employees were exposed to amputation hazards.

Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.
