29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).    a. On or before May 6, 2015, an employee operated a forklift without having completed training and evaluation, thereby exposing employees to struck-by hazards.    b. On or before May 6, 2015, an employee operated a yard truck without having completed training and evaluation on that specific piece of equipment, thereby exposing employees to struck-by hazards.    c. On or before May 6, 2015, employees operated powered industrial vehicles, other than that in which they were trained and evaluated on, thereby exposing employees to struck-by hazards.    Schwarz Supply Source was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1)(ii), which was contained in OSHA inspection number 190371, citation number 1, item number 1 and was affirmed as a final order on May 9, 2012, with respect to a workplace located at 6000 Greene Pointe S. Dr. Groveport, OH 43125.
