29 CFR 1926.452(j)(2): Poles of pump jack scaffold(s) were not secured to the structure by rigid triangular bracing, or equivalent, at the bottom, top and other points as necessary: a) Lee Allen Contracting, Inc. DBA Allen Roofing @ 1415 Colonial Drive, Cottleville, MO 63304: On May 8, 2015, the employer did not ensure that the Alum-A-Pole pump jack scaffold was secured at the bottom. The system was erected with 5 poles and measured approximately 52 feet in length. One pole had visibly sunk into the dry dirt and another pole utilized a cracked 2x10 mud sill, which was made level with a single rock underneath, to level the mounting surface for the pole.
