29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a) Grand County Roofing and Sheet Metal, Inc. dba The Roofing Company: On or before May 5, 2015 the exposing employer did not ensure that employees were protected from injury, in that several employees removing and installing shingles on the first and second levels on two-story residential units were not utilizing a fall protection system. The pitches of the roofs were 6-in-12. This condition exposed employees to falls ranging from 10 to 25 feet.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: (a) Grand County Roofing and Sheet Metal, Inc. dba The Roofing Company: On or before May 5, 2015 the exposing employer did not ensure that employees used the proper attachment point for their body harness in that employees were utilizing the side attachment point on their body harness. This condition exposed employees conducting roofing operations to hazards associated with improper use of personal fall arrest systems.
