29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b). (a) Grand County Roofing and Sheet Metal, Inc., DBA The Roofing Company, at 675 Wapati Drive, Frasier CO 80442: On or before July 7, 2020, the employer did not ensure their employees were protected from injury, in that employees actively engaged in roofing activities on top of a residential home, with a roof pitch of 6/12-12/12, were not utilizing a fall protection system. This condition exposed employees to a fall hazards of 15-30 feet.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: (a) Grand County Roofing and Sheet Metal, Inc., DBA The Roofing Company, at 675 Wapati Drive, Frasier CO 80442: On or before July 7, 2020, the employer did not ensure that employees used the proper attachment point for their body harness, in that employees were utilizing the front attachment point on their body harness. This condition exposed employees conducting residential roofing activities to hazards associated with improper use of personal fall arrest systems.
