29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:      Location-Roof, 59 Andrews Pkwy, Devens, MA:  On or about May 4, 2015, employees replacing roof shingles damaged by snow and ice build up over the winter was not protected from a fall of approximately 35 feet where personal fall arrest equipment was not utilized.
