29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.  a) Building Exterior/Side:  The end of the platform was not designed and installed so that the cantilevered portion of the platform was able to support employees without tipping, the Alum-A-Pole scaffold had an 3 foot overhang over the support, on or about 1/28/2020.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  a) Building Exterior/Rear: An employee was exposed to a 16 foot fall hazard while working from an alum-a-pole pump jack scaffold that was not equipped with a guardrail system or other means of acceptable fall protection in place, on or about 1/28/2020.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) Building Exterior/Side: An employee working without hard hat was exposed to electrical burns and shock while engaged in siding instillation next to an energized electrical line, on or about 1/28/2020.  The Precision Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1275250, citation number 1, item number 1 and was affirmed as a final order on 12/26/2017, with respect to a workplace located at 749 Marin Ave Lyndhurst, NJ 07071.
