29 CFR 1926.302(b)(3): All pneumatically driven nailers, staplers, and other similar equipment provided with automatic fastener feed, which operate at more than 100 p.s.i. pressure at the tool shall have a safety device on the muzzle to prevent the tool from ejecting fasteners, unless the muzzle is in contact with the work surface:    (a) Front porch roof:  On or about 05/07/15 - Employees were exposed to struck-by hazard in that was using a Pneumatic nailer with the automatic safety disconnected. The safety device was wired open with copper wire.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    (a) Front porch roof:  On or about 05/07/15 - Employees were exposed to a fall hazard of 8 feet in that they were working from a low slope roof installing siding.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support:    (a)  Front porch roof:  On or about 05/07/15 - Employees are exposed to a fall hazard in that the extension ladders being used by an employees, while installing siding, was not extended at least 3 feet above the landing of the porch roof.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a) Front porch roof:  On or about 05/07/15 - Employees were eye injury in that they were using a pneumatic air nailer without eye protection.    (b) Front porch roof:  On or about 05/07/15 - Employees were exposed to an eye injury in that they were wearing regular sun glasses while operating the pneumatic nailers.    The Santamaria Siding LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.102(a)(1), which was contained in OSHA inspection number 982570, citation number 1, item number 1 and was affirmed as a final order on 09/30/14, with respect to a workplace located at 8 Addison Park Dr., Huntsville, AL 35801.
