29 CFR 1910.132(a): Protective equipment was not maintained in a sanitary and reliable condition: On May 6, 2015, at the TimkenSteel Harrison Plant continuous caster area employees were exposed to a fall hazard of approximately 12 feet while using cutting torch lances to melt through and remove the solidified hot metal mounds on top of the steel grating that covers the floor level below. The employee fall protection harnesses were not properly adjusted to maintain proper physical restraint.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On May 5, 2015, at the TimkenSteel Harrison Plant tundish repair area, employees were exposed to a concrete mixing machine flywheel that had damaged guarding. There was an ingoing nip point with an approximate opening of 1 inch by 3 inches wide.
