All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). (a) Employees performed die changes on the hydraulic presses without first isolating the electric, hydraulic and gravity energy sources. (b) Employees changed the hammers on the knockout machines without first isolating the electric and pneumatic energy sources. (c) Employees changed the blades on the vertical band saws without first isolating the electrical energy sources. Seilkop Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 279834, citation number 1, item number 2a and was affirmed as a final order on April 25, 2013, with respect to a workplace located at 425 W. North Bend Rd., Cincinnati, Ohio.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA: (a) The employer did not report an employee's amputation within 24 hours of the incident, in that on April 17, 2015 an employee suffered an amputation at the company that was not reported until May 4, 2015.
