29 CFR 1926.760(b)(3): Each connector working at heights over 15 and up to 30 feet above a lower level, was not provided with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or was not provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section: (a) Jobsite, Roof: On or about May 12, 2015, the six employees working from the roof installing roof panels were not provided with a personal fall arrest system, fall restraint system, or any other means of protection from fall hazards. This fall protection equipment was not on the site let alone installed and available for use on the roof. The fall hazard to the employees ranged from 24 feet to 25 feet 10 inches.
