29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a). Exterior of the Building - On or about and times prior to May 13, 2015, the employer did not provide training that addressed safe usage of a Genie Articulating Boom Lift (model: Z-45/25) to employees who used the lift to install exterior sheathing, thus exposing employees to a fall hazard of up to 18.5 feet onto earthen ground.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.    a). Exterior of the Building - On or about May 13, 2015, personal fall protection equipment was not worn by employees installing exterior sheathing while working from a Genie Articulating Boom Lift (model: Z-45/25), thus exposing employees to a fall hazard of up to 18.5 feet onto earthen ground.    Vision Contractors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.453(b)(2)(v), which was contained in OSHA inspection number 617340, citation number 1, item number 4 and was affirmed as a final order on October 10, 2012, with respect to a workplace located at 1191 Kernan Boulevard, Jacksonville, FL 32225.
