Exterior of the Building - On or about and times prior to May 13, 2015, the employer did not provide training that addressed safe usage of a Genie Articulating Boom Lift (model: Z-45/25) to employees who used the lift to install exterior sheathing, thus exposing employees to a fall hazard of up to 18.5 feet onto earthen ground.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. a). Exterior of the Building - On or about May 13, 2015, personal fall protection equipment was not worn by employees installing exterior sheathing while working from a Genie Articulating Boom Lift (model: Z-45/25), thus exposing employees to a fall hazard of up to 18.5 feet onto earthen ground. Vision Contractors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.453(b)(2)(v), which was contained in OSHA inspection number 617340, citation number 1, item number 4 and was affirmed as a final order on October 10, 2012, with respect to a workplace located at 1191 Kernan Boulevard, Jacksonville, FL 32225.
