A suitable eye wash station was not provided in that the eye wash station was not maintained in the upright position where both eyes can be flushed immediately if the event of a corrosive chemical splash, on or about 5/14/15.

The employee had work restrictions for 12 days. The injury was not recorded in the OSHA 300 log within seven days, on or about 5/14/15.
