29 CFR 1910.95(c)(1): A continuing, effective hearing conservation program as described in 29 CFR 1910.95( c) through (n ) was not instituted when employee noise exposures equaled or exceeded an 8 hour time weighted average sound level (TWA) of 85dBA:            Legacy Converting, Inc / Cranbury NJ: The Employer did not provide a continuing effective Hearing Conservation Program for employees exposed to noise above 85 dBA. Employees were exposed to continuous noise levels equivalent to 75.36%  (87.9 dbA) and 129.8% (91.8 dbA) respectively of the allowable dose of 50%, or 85 dBA for an 8-hour TWA. The levels are above the 8-hour TWA of 85dBA over 400 minutes. Zero exposure was assumed for all unsampled time periods.     Observed on or about 05/15/2015.

29 CFR 1910.1200(h)(1): Employers were not provided information and training as specified in 29 CFR 1910.1200(h)(2) and (3) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area:          Legacy Converting, Inc/ NJ site: Employees exposed to chemicals which include, but not limited to Ceram Clean II product, Super Tech GL5 80W90 Gear oil, and Klean Strip lacquer thinner did not receive site specific training of the risks associated with products handled during cleaning/maintenance operations.   Observed on/or about 05/15/15.
