29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use. On or about May 8, 2015, where employees are exposed corrosive chemical: a. in the warehouse, battery charging area, VTC, the shower portion of the eyewash station worked but the handle that operated the basin had been removed. b. in the southeast corner of building II, Battery charging area, the eyewash is not adequate for a fifteen minute flush/shower of the eyes and/or body.

On or about May 8, 2015 in the Maintenance VTC, employees were exposed to a struck by hazard when: a. the Mizuho pedestal grinder's adjustable tongue guard on the top of the grinder used was kept with an opening greater than 1/4 of an inch. b. the JET bench grinder adjustable tongue guard on the top of the grinder used was kept with an opening greater than 1/4 of an inch.
