29 CFR 1926.451(e)(1): Scaffold platform more than 2 feet above or below point of access, was not provided with a ladder or direct access from another structure or other equivalent safe access during use: a) 70 College Avenue, New Brunswick, NJ 08901. Employees were exposed to fall hazards when a ladder was not provided to access the walking/working platform of the scaffold. Condition occurred on ro about 05/18/15.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of walking/working platforms: a) 70 College Avenue, New Brunswick, NJ 08901. Employees were exposed to fall hazards while working on and from tubular welded scaffold when the guardrail system they were using for their fall protection was inadequate in that the top and mid rails were not at the correct height. In addition, they had installed a 1/2 inch rope in some areas as their top rail. Condition occurred on or about 05/18/15.
