29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a- 5631 Highway 9 North, Howell NJ: Employees, not supplied with fall protection, were exposed to fall hazards in excess of 13 feet to the ground surface below when performing exterior brick application and other masonry related activities from a prefabricated, tubular welded framed scaffold whose walking/working surface was not fully planked, was missing guardrails on its active level, contained only one end on its upper active level that wasn't braced or plumb, etc. In addition, the worksite company employees had not received any scaffold hazard recognition training from the company. Condition observed on or about 04/13/16. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.
