29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Shipping/Receiving Area: Employees conduct energy control procedures on machinery such as but not limited to a Harmony bailing machine, s/n M60CB-0109, which has an electrical energy 460/480 three phase source. Servicing and maintenance was being performed on the baler and a lock was not applied to the energy isolation device, as determined on or about 05/14/2015. ABATEMENT CERTIFICATION REQUIRED
