29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. a) On or about May 12, 2015, and at times prior, Roofcraft, Inc., as a controlling employer, did not ensure that employees of Steve Holcomb Roofing, Inc. (subcontractor) used body harnesses that had attachment points in the middle of the back, near shoulder level. Employees used body belts for fall arrest with attachment points at the waist line.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure: a) On or about May 12, 2015, Roofcraft, Inc., as a controlling employer, did not ensure that Steve Holcomb Roofing, Inc. (subcontractor) protected their employees from falls greater than 6 feet while performing roofing work without adequate fall protection measures in place. Roofcraft, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 986137, citation number 1, item number 1 and was affirmed as a final order on August 12, 2014, with respect to a workplace located at 801 Zerex Street, Fraser, CO 80442.
