Lathrop Electric, Inc., 350 East 6th Street, Erie, PA  16507, first floor storage area, on or about May 14, 2015 and times prior thereto:  Electrical inspections of flexible cords and extension cords were not adequate to identify damage and defective parts as evidenced by:  1.)  Electrical equipment included, but was not limited to, the flexible electrical cord on the Ridgid Wet/Dry 16-gallon vacuum.  The flexible electrical cord was frayed at the cord cap and near the termination into the vacuum.    2.)  Electrical equipment included, but was not limited to, a yellow flexible electrical extension cord was frayed.

Lathrop Electric, Inc., 350 East 6th Street, Erie, PA  16507, north interior stairwell, on or about May 14, 2015 and times prior thereto:  Employee were utilizing a set of interior stairs on the north side of the structure to access upper and lower floors.  A metal electrical knockout box was hanging in the stairwell area by non-metallic sheathed cable.  The metal electrical knockout box was potentially exposed to physical damage and snaring in that the box was not secured and mounted.

Lathrop Electric, Inc., 350 East 6th Street, Erie, PA  16507, north interior stairwell, on or about May 14, 2015 and times prior thereto:  Employee were utilizing a set of interior stairs on the north side of the structure to access upper and lower floors.  A metal electrical knockout box was hanging in the stairwell area by non-metallic sheathed cable.  The metal electrical knockout box was potentially exposed to physical damage and snaring in that the box was not secured and mounted.  B.)  Lathrop Electric, Inc., 350 East 6th Street, Erie, PA  16507, north interior stairwell, on or about May 14, 2015 and times prior thereto:  Employee were utilizing a set of interior stairs on the north side of the structure to access upper and lower floors.  A metal electrical knockout box was hanging in the stairwell area by non-metallic sheathed cable.  The non-metallic sheathed cable was potentially exposed to physical damage and snaring in that theType NM cable was not secured.
