29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees performed manual scraping of coating from pipe rotating on a conveyor to an abrasive blasting booth. The employees placed hands over a guardrail for scraping, placing hands in the area of the conveyor and rotating pipe, exposing employees to caught-in hazards.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. An employee had sufferred from an amputation during manual scraping of pipes on May 1, 2015. The employer had not reported the amputation to OSHA within twenty-four hours.
