29 CFR 1910.132(d)(1)(I): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE), nor did the employer select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: (a) Near the Slide Gate for the South Lime Elevator Chute, under the discharge end of Lime Kiln, in the Recausticizing Area of the Pulp Mill - On or about May 16, 2016 an employee was using a plant utility water hose attempting to wash back a ~3 foot deep pile of accumulating quicklime (calcium oxide) in order to advance to and close the south bucket elevator slide-gate. Hot lime was leaking from a hole in the 1st Reburn Overflow Pipe (located between the Day Bin and the Reburn Lime Silo) at a point about 40-50 feet above ground and was accumulating fast near the slide gate. No comprehensive workplace hazard assessment had been conducted for the Lime Kiln System, nor had appropriate protective foot-ware been recommended for Utility employees working in this area.
