29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed to prevent moisture or water from entering and accumulating within the enclosures: (a) Wet-End stock - On or about May 26, 2016 spilled wet hot stock material lands directly onto a 480 volt electrical panel box mounted on the block wall.

29 CFR 1910.27(f): "Maintenance" All ladders were not maintained in a safe condition. All ladders were not inspected regularly, with the intervals between inspections being determined by use and exposure. (a) Wet-end stock- On or about May 26, 2016, the two fixed ladders used by employees to gain access to the upper level platforms has a side support leg that was rusted and broken loose from the anchor on the concrete floor.
