29 CFR 1926.501(a)(2): The employer did not determine if the walking/working surfaces on which its employees were to work on, had the strength and structure integrity to support employees safely:  On or about 5/15/2015 at 2149 Fothergill Drive in Evans, Georgia: Employees engaged in wood framing on the second level walking/working surface near the access ladder of the single-family home were exposed to a fall hazard of up to 21 feet 6 inches. One employee while accessing the second level was standing on a section of compressed plywood that was not secured and did not support the weight of the employee causing him to fall head first from the upper level to the concrete basement floor.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 5/15/2015 at 2149 Fothergill Drive in Evans, Georgia: Employees engaged in wood framing on the single-family home were exposed to fall hazards at heights up to 21 feet 6 inches to the ground level.  The employer failed to protect the employees from fall hazards such as but not limited to:  1. Open sides, and ends on the first and second levels of the home. 2. Door opening on the first level of the home. 3. Several window openings were missing a top rail on the first level of the home. 4. Basement floor opening on the first level of the home.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about 5/15/2015 at 2149 Fothergill Drive in Evans, Georgia: Employees were exposed to fall hazards while performing framing work at a single-family home at height up to 21 feet 6 inches from the ground. The employer failed to provide fall protection training to all employees who were exposed to fall hazards.
