29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) East side, new residential construction located at 4956 Kentwood Drive, Marietta, GA 30068: Employees using pneumatic Hitachi Finish, Framing, and Siding Nail Guns were exposed to eye injuries while operating these tools without protective eyewear. Employees operating these tools at 120 PSI were exposed to struck-by hazards which included plastic and metal collated debris and fasteners as safety glasses and or goggles were not worn. (b) Front side, new residential construction located at 4956 Kentwood Drive, Marietta, GA 30068: Employee using a Makita (Model 5007F) 7-1/4-inch Circular Saw was exposed to eye injuries while operating this tool without protective eyewear.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: (a) Front side at cut bench, new residential construction located at 4956 Kentwood Drive, Marietta, GA 30068: Employee was exposed to severe lacerations and amputation hazards while operating a Makita (model 5007F) 7-1/4-inch circular saw without the lower guard in operation. A wood shim was placed between the upper and lower guards which prevented the lower guard from retracting automatically after a cut through the materials was made.

The employer failed to inspect the plug and all electrical equipment on site prior to the start of work.
