29 CFR 1910.136(a): The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards:   Load and Unload Areas:  On or about 5/20/15, the employer did not select and require package handlers to wear impact protective shoes to protect them from the foot hazards identified in the PPE hazard assessment for their job.  A package handler had a fractured toe injury on 2/10/15 when a heavy package dropped on his foot.
