29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Workshop: On or about May 18, 2015 and at times prior thereto, the Wilton Drill Press - S/N - 2100260, did not have a guard at the chuck, exposing employees to a rotating hazard at the point of operation. The drill is used on wood or metal as aluminum and steel.

29 CFR 1910.213(p)(4): Belt sanding machines shall be provided with guards at each nip point where the sanding belt runs on to a pulley. These guards shall effectively prevent the hands or fingers of the operator from coming in contact with the nip points. The unused run of the sanding belt shall be guarded against accidental contact. Workshop: On or about May 18, 2015 and at times prior thereto, the Grizzly Industrial Edge Sander did not have a guard at the back, exposing employees to a rotating hazard at the point of operation. The unguarded area measured approximately 32 inches in length. The guard is used to protect operators from flying wood and plastic hazards while using the edge sander.

29 CFR 1910.303(g)(1)(i): The dimension of the working space in the direction of access to live parts operating at 600 volts or less and likely to require examination, adjustment, servicing, or maintenance while alive was less than indicated in Table S1: Old Chiller Room: On or about May 18, 2015 and at times prior thereto, the electrical panels with shut off control providing power to the ECHWP-1 Exchange Pump was blocked by general purpose containers, a chained vacuum cleaner. The space was used for storage, thereby preventing direct access with 3-foot minimum clearance. Workshop: On or about May 18, 2015 and at times prior thereto, the electrical panels with shut off control providing power to the compressor - Manufacturer: Westward was blocked by a work stand, bucket, cardboard and miscellaneous supplies. The compressor supplies air pressure for pneumatic tools. The space was used for storage, thereby preventing direct access with 3-foot minimum clearance.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): Workshop: On May 18, 2015 and at times prior thereto, (1) the Dewalt Chop Saw #45760 had damaged insulation on the cord at the plug and receptacle; (2) a flexible extension cord (orange) had damaged insulation and was used to power tools; (3) a flexible extension cord (yellow) was missing a ground pin. The cord is used to provide power for motor and hammer drills, exposing maintenance workers and operators to electrical shock hazards.
