29 CFR 1910.147(a)(2)(ii): Alternative measures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by the exception of 1910.147(a)(2)(ii) for routine, repetitive, and integral processes such as tool change, change over, and cleaning: a) Embossing section of the #4 Perini Winding Machine - On or about May 18, 2015, alternative measures of protection were not utilized while threading the machine allowing for employee exposure to an amputation hazard between the marrying roller and a steel roller.
